Definition To provide supportive and direct services to mentally ill clients as outlined in the individual plan of care and directed by the primary clinician or treatment team. DISTINGUISHING CHARACTERISTICS This is the entry level position in the Mental Health Case Manager class series. Incumbents are involved in providing day-to-day case management duties under close supervision. Incumbents may transport clients to and from treatment, aid the client in obtaining personal needs items, provide support for other case managers and the clinical team. SUPERVISION RECEIVED AND EXERCISED Immediate supervision is provided by a Clinician, Unit Manager, RN, higher level Mental Health Case Managers, or other licensed clinical staff. Typical Duties DUTIES may include, but are not necessarily limited to: Aid in implementation of case plans to resolve clients' problems and needs concerning the necessities of day-to-day living. Coordinate and advocate for social services to maximize support systems, avoid duplication, and minimize risk of homelessness. Teach behavioral/community functioning skills such as cooking, cleaning, accessing public transportation, and time management to enable clients to gain more independent living skills. Assist with coordination of placement, conduct home visits, and provide limited transportation to mental health services, family visits, and collateral contacts. Assist with desk duties and process paperwork. Assist clients in arranging transportation to and from treatment programs, maintain order in the vehicle, observe and note client behavior while being transported, and maintain daily mileage log. Assist therapists and other case managers in carrying out activities described in treatment plan. Assist with placement determinations. Maintain daily records of client contacts, activities, and mileage. Perform related duties as assigned.(Essential duties may vary from position to position within this classification. Employment Standards NECESSARY EMPLOYMENT STANDARDS Knowledge of: Math sufficient to use measuring instruments, maintain and balance a checkbook, create simple budgets, and add receipts. Programs and treatment techniques for the support and care of the mentally ill. Concepts of mental illness and mental health: the physical, emotional and social needs of mentally or emotionally disordered individuals. Case management methods, practices, policies and procedures relating to the provision of services to the mentally ill. Cultural and socio-economic factors and influences affecting mental health. Community resources available for food, shelter, emergency services and programs. Basic social service principles, practices, and programs. Skill/Ability to: Retain and recall information pertaining to specific procedures. Understand and follow verbal and written instructions. Use patience, tact and courtesy when dealing with clients and their families, co-workers, and the general public. Keep legible and accurate records and files. Communicate effectively with others. Maintain effective working relationships with co-workers and other departments. Adjust to changes in workload and assignments. Maintain confidentiality of information about patients. Lead and interact with clients/patients in social, recreational and living skills activities. Present oral and written reports concisely and clearly. Write clear, concise, grammatically correct reports, records and letters. File alphabetically and numerically. Education and Experience: Any combination of education and experience that could likely provide the required knowledge, skills, and abilities is qualifying. A typical way to obtain the knowledge, skills, and abilities would be: Education: Equivalent to completion of the twelfth grade. Supplemental coursework in the behavioral, biological or social sciences, or community college certification in the areas of Human Services, Social Work, or Home Health Aide are desirable. Experience: Six months of experience in social services, mental health, or human services field which includes experience with teens or adults.
