This position involves discharge planning interventions with patients/families under the direction of Care Coordinators and Clinical Social Workers. Under the direction of the clinical staff, the Discharge Planner performs multiple activities that support the movement and transitions of patients out of the inpatient hospital setting, using current knowledge of hospital processes and community services. It involves frequent contact with patients and families, as well as multiple hospital staff and community resource, agency, facility and/or payor representatives. Acts as an internal resource to the Business Office around discharge planning, serves as a liaison to external case managers, review organizations, insurance companies, around discharge planning. This position requires flexibility of days and hours.

For BSW: One year experience in a health care setting or if a new graduate, field placement in a health care setting.

For Bachelors in a closely related field: Recent experience of more than three years in case management and/or utilization management is required.

For LVN: Recent experience of more than three years in case management and/or utilization management is required.

Bachelors in Social Work or closely related field with experience listed below, or Licensed Vocational Nurse with experience listed below.


MAB (Management of Assaultive Behavior) required within 30 days of hire/transfer. Must be renewed annually.


Exemplary communication, coordination and problem-solving skills, with emphasis on customer service toward patients, families, community providers and payors. Type minimum of 40wpm preferred. Strong computer skills. Ability to manage activities and multiple priorities within complex systems. Tolerance for ambiguity, uncertainty and change. Ability to function in unstructured environment.
